Please complete all fields and upload the requested documents (in PDF or JPG format) to create a new ML Schedules™ Software user account and related Group(s).



Class Description
District Group For LCSD employees, student clubs, departments, etc.
District Related Groups Affiliated groups such as Boosters and PTSO.
Government Entity Local, State or Federal Agency or Official conducting official business (Insurance Required)
LCSD Athletics District Athletic Department and Sports Teams.
Non-Profit Groups All non-profit groups, community youth groups, and church groups. (Insurance Required)
Private Groups Private parties, commercial, business and profit-making organizations. (Insurance Required)

Group Information

If you are an internal staff member who will be submitting requests on your own behalf (i.e. not for a group, team, club, etc), use your first and last name as the Group Name.

Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

A Certificate of Insurance or other proof of insurance in a form acceptable to the District is required of all outside groups requesting use of District facilities. The District will be listed as an Additional Insured for a minimum amount of $1,000,000 each occurrence and $2,000,000 general aggregate. This form can be obtained from your insurance carrier.  The additional insured/ certificate holder address should be: Liverpool Central School District, 195 Blackberry Road, Liverpool, NY 13090.
